Cauda Equina Syndrome

Cauda equina syndrome is a surgical emergency that arises from the compression of the cauda equina, the bundle of nerves at the lower end of the spinal cord.

The main causes include herniated discs, spinal stenosis, tumors, infections, trauma, and inflammatory conditions. These conditions lead to the narrowing of the spinal canal, which then compresses the cauda equina nerves.

Patients typically present with severe lower back pain, sciatica, numbness or tingling in the saddle area (buttocks, inner thighs, and perineum), and weakness in the lower extremities. Bowel and bladder dysfunction, such as incontinence or retention, are also common and significant symptoms.

The pathophysiology involves disruption of both sensory and motor pathways due to the compression of the cauda equina nerves, these nerves control the lower limbs and pelvic organs. Compression leads to a loss of function in these areas, resulting in the symptoms mentioned earlier. The severity of nerve damage depends on the extent and duration of the compression.

It’s a surgical emergency, because delayed treatment can lead to permanent neurological deficits. The goal is to quickly decompress the nerves to restore function and prevent irreversible damage. Delays can result in chronic pain, paralysis, and permanent  loss of bowel and bladder control. Decompression endoscopically is preferred choice in cauda equina syndrome.

Endoscopic spinal surgery is a minimally invasive technique. In endoscopic spinal surgery  an endoscope is used to operate, which is a small camera with surgical instruments, to access the spine through a small incision. This allows decompression of the affected nerves with minimal tissue damage. The benefits of this approach include reduced postoperative pain, shorter hospital stays, and faster recovery times compared to traditional open surgery.

Early diagnosis, urgent surgical intervention, and the use of endoscopic techniques are key to managing cauda equina syndrome.
